REQUIREMENTS FOR AUTHORIZING SOMEONE TO PICK UP ORIGINAL DOCUMENTS
Any person intending to pick original documents on behalf of another MUST present the following; A. Formal letter from a nurse/midwife to the Registrar authorizing the release of documents – clearly specifying the relationship with the person authorized to pick the documents and the reason (s) for the inability to collect documents in person with evidence attached.
B. Powers of Attorney – duly commissioned or notarized as the case may be. It should specify the following;
Name of the person authorizing the release of documents (Donor)
Name of person authorized to collect documents (Donee)
Physical address and contacts of both the Donor and Donee
L.C letter from the area of residence of the Donee
National Identity cards (photocopies) for both parties which must be signed on
C. Original professional and academic documents of the Donor D. Video call with UNMC secretariate to confirm personality/identity
