To replace a lost document (a practicing license or certificate of enrollment/ registration, you are supposed to write a formal application to the Registrar, requesting to replace the lost document. The following attachments must accompany the request;
A. Police letter B. Letter from Nursing/Midwifery Training School, with a passport photo attached. C. Nursing School ID and Final Semester Examination Card D. Testimonial from Nursing School E. UCE certificate and result slip (UACE certificate and result slip where applicable) F. Secondary school ID G. UNMEB certificate and UNMEB transcript/ result slip H. Proof of advert of loss of document in an authentic newspaper (2 weeks after the publication) I. Send a soft copy of a passport photo by email – certificates@unmc.ug J. Any other professional/ academic/ state documents showing that you own the lost documents. K. Proof of replacement fee payment
